Note that we require any and all co-payments prior to services performed. If you do not have your co-payment with you, we will assist you in re-scheduling your appointment.

Due to excessive requests for completion of paperwork on behalf of the patient, we have implemented a $25 fee for this service. We will gladly fill out one form as a courtesy, but will charge the $25 fee for any additional pages.
